Make a referral

We accept referrals from local authorities, social workers, housing officers, and support agencies. This form is for professional use only — an initial assessment is completed before every placement is confirmed.

Who can refer

Social workers, housing officers, probation officers, NHS and community health teams, and voluntary sector organisations. We do not accept self-referrals.

Eligibility criteria

Single adult aged 18+, currently homeless or at risk. The client must have capacity to engage with supported housing, or a supported decision-making arrangement must be in place.

What happens next

We aim to respond within 2 working days. A team member will contact you to discuss the referral and arrange an initial assessment. No placement is made without this assessment.
